Stephenville Crossing: Work with a representative from Service Canada to apply for the Canadian Dental Care Plan. This program requires registration by email (stephenvillecrossing@nlpl.ca) or phone (709-646-2173). Please let us know if you are applying for yourself only, or multiple people, and we will schedule a time for you to come to the library for your application process.

You must bring:
– One valid government ID (such as driver’s license)
– Social Insurance Number (SIN) (if available for children)
– date of birth
– full name
– home and mailing address
– list of dental coverage through government social programs (if applicable)
– applicant and their spouse or common-law partner (if applicable) must have filed their 2025 tax return in Canada and have received their notice of assessment
